Embracing Vermont Initiative:
The Embracing Vermont Initiative, led by the Vermont Village Community Development Corporation (VVCDC), is a transformative, multi-year community-driven initiative aimed at advancing economic opportunity, workforce development, housing, and community revitalization along the Vermont Corridor. This initiative, which is designed to maximize partnerships with like-visioned organizations in the region, seeks to align with the Weingart Foundation, ensuring long-term, measurable impact through cross-sector collaboration.
VVCDC is committed to delivering sustainable, community-driven change and are eager to formalize a partnership with the Weingart Foundation. Our goal is to avoid duplicating existing services, leverage current efforts, and ensure that local residents directly benefit from public and private investments.
FreshHome Housing Navigation:
FreshHome provides housing navigation services to help individuals and families secure safe, affordable housing. Housing is a fundamental need and a critical determinant of overall health and stability. FreshHome assists residents with navigating the housing market, accessing rental assistance programs, and finding long-term housing solutions. This program is essential for ensuring that individuals have a stable home from which they can focus on improving their health, employment prospects, and overall quality of life.
CalFresh:
CalFresh ensures that low-income families and individuals have access to food assistance, reducing food insecurity and providing them with the resources to maintain good nutrition. This program directly impacts the success of ECM by ensuring that families can allocate their resources to other critical needs, such as healthcare and housing, while maintaining their health through proper nutrition.
Fresh Path:
Food Services Referral, Health Meal Plans, Community Gardening: Fresh Path addresses food insecurity by providing referrals to local food services, meal planning support, and access to community gardening initiatives. In South LA, food insecurity is a significant barrier to health, and Fresh Path works to provide residents with access to nutritious food, as well as teach them how to maintain a healthy diet. Through community gardening, individuals not only gain access to fresh produce but also engage in community-building activities that promote long-term resilience and self-sufficiency.
Workshop:
Financial Literacy
Financial literacy is a cornerstone of self-sufficiency. Our Budgeting and Couponing Workshops equip participants with the skills to manage their finances, save money, and reduce expenses. This program is particularly beneficial for those facing financial challenges, such as individuals reentering society after incarceration, as it empowers them to better manage their finances and achieve long-term economic stability.
FreshCare Link:
ECM Case Management and Referrals: The Fresh Care Link program provides vital Enhanced Care Management (ECM), Community Health Work (CHW)linking residents to a broad spectrum of services—healthcare, housing, mental health support, and more. This program ensures that individuals, particularly those with complex needs, are connected to the appropriate resources to improve their quality of life. Fresh Care Link is an essential part of the community-based model, supporting residents as they navigate social and health systems and helping them gain access to the services that will enable them to live healthier and more stable lives.
